Either way is possible, since none are proven or disproved. They're not "scripted" but just manipulated. They could just as well be real personalities who just forgot about their previous lives. Same for their personalities being fake. It could be her faking it, it could be her having the wrong memories of who is "fictional", etc.) Either the DR1, DR2, UDG, DR3 universe is fake and this is the 53rd game, or Tsumugi simply lied about it because she wanted to bring despair to the survivors.
